myHealthButton is not responsible for any data usage, overages, or other carrier charges associated with the data usage whether the myHealthButton is used over Wi-Fi or the cellular data network.
To open an account, you must complete the registration process by providing us with current, complete, and accurate information as prompted by the Registration Form. You will then choose a password. You are entirely responsible for maintaining the confidentiality of your password and account. Furthermore, you are entirely responsible for any and all activities that occur under your account. You agree to notify State of Michigan (SOM) immediately of any unauthorized use of your account or any other breach of security. Do not give out your password or allow it to be seen, or used, by anyone else. If someone else learns your password, they might use this Application to get private information about you without your knowledge that we cannot stop until you change your password.
To use the Application, you must accept the terms of the AGREEMENT. By tapping on the "I ACCEPT" button, you are agreeing to the terms of this AGREEMENT.
You represent that you are of sufficient legal age to use this Application. We reserve the right to terminate your use of the Application at any time, with or without reason.
The Application is provided solely for informational purposes and reliance on the Application is purely at your own risk. SOM does not warrant the accuracy or completeness of the content contained in the Application. You will be solely responsible for your use of and reliance on the Content contained in the Application and FOR ALL decisions or actions resulting from your use of the Application.
The Application is not a substitute for speaking with your Authorized Representative, case worker or Provider.The Application is not intended to replace or overrule any diagnosis assigned by the doctor and must not be used as the basis for making any diagnosis, treatment recommendation, or other health decision.

The term of this AGREEMENT shall be the date on which you tap the "I ACCEPT" button until terminated. You may terminate this AGREEMENT at any time by deleting your account in this Application. This AGREEMENT and your right to use the Application will terminate immediately without notice from us if you fail to comply with any provision of this AGREEMENT.
In connection with use of the Application, you may from time to time receive updates to the Application from CNSI which may be automatically downloaded and installed to your device. These updates may include bug fixes, feature enhancements or improvements, or entirely new versions of the Application. You agree that CNSI may automatically deliver such updates to you for use of the Application and you shall receive and install them as required. SOM may also make changes to this Agreement without notice to you. You should review this Agreement occasionally to see if there have been any changes that may affect you. Your continued use of this Application signifies your agreement to any updates to this Agreement.
The Application may include certain features or services that rely upon device-based location information obtained from Global Positioning System (GPS) technology, trackers, or other location tracking tools on your mobile device that may collect, use, transmit, process and maintain your location data, including the real-time geographic location of your device (such as your device ID and name, device type and real-time geographic location of your device at time of your request). You hereby agree and consent to Our and Our partners and licensors collection, use, transmission, processing and maintenance of such location data solely to provide such services. You may withdraw this consent at any time by not using the location-based features or by turning off the location services settings (as applicable) on your device. Your use of any third party services that use or provide location data as part of the Application is subject to such third party's terms and privacy policy on its use of location data. Neither We nor any of Our partners and licensors guarantee the availability, accuracy, completeness, reliability, or timeliness of location data or any other data displayed by the Application. Any and all use of real-time geo-location features is at your sole and exclusive risk.
In order to use many of the features of this Application, including features which allow you to access your Healthcare information, you will need to have a mihealth card and be an active Medicaid recipient. If you are not a current Medicaid recipient, please visit your SOM Medicaid office to request Medicaid benefits and possibly obtain a valid mihealth card. All of the information accessed, used, stored and submitted on or through your myHealthPortal or myHealthButton account is subject to the Privacy Policies of the SOM.
Not all of your healthcare information will be available on this Application. The data collected by this Application is transmitted to CNSI and securely stored on CNSI's servers in the United States. We will not share this information with any third party in identifiable form. We may use or disclose aggregate and de-identified data collected during your use of the Application for research, educational, and product development purposes, and to solicit user comment and feedback, and in connection with publications. The de-identified information could never be used to identify or contact you.
We reserve the right to amend this policy at any time by posting changes on our website and updating the effective date of the policy to reflect the date of the changes. By continuing to use the Application after we post any such changes, you accept the Privacy Policy as modified.
